Value and discounts are based on your dentist’s standard professional fees.
Our plan is not insurance or a qualified plan under the Affordable Care Act.
The membership plan includes a 30-day money back guarantee. You can receive a full refund up to 30 days after purchase if no services or discounts have been used.
Payment for treatment(s) not included in the plan is due at the time of service.
If paying monthly, you must pay the first three months up front when initially purchasing the plan. You will begin the monthly payments after the third month.
Plan discounts do not apply to the following procedures completed at our practice:
· Cosmetic treatment such as cosmetic bonding and veneers
· Implant Services (CDT Codes D6000-D6199)
· Orthodontics such as comprehensive orthodontic treatment and appliance therapy (CDT Code D8000-D8999)
Our membership plan does not cover procedures completed by specialists outside our practice.
